All information is for office use only.

            A copy of the insurance card that is provided is given to the athletic trainer and the coach as well as a copy kept on file in the athletic office.

            Insurance card copies and any information kept in the athletic office is shredded at the end of each school year.

The Athlete Name, Birthdate, Grade, and Years of participation are provided to PIAA for eligibility purposes.

The PIAA Physical forms are kept with the Athletic Trainers
